TSDOH Assessment

The TSDOH (Technological Social Determinants of Health) Assessment Matrix is a proprietary 36-question assessment tool developed by Dr. Troy Nash that measures technology readiness across six domains: Access, Literacy, Trust, Support, Adoption, and Outcomes.

It's the first validated framework that treats technology access as the 6th social determinant of health—recognizing that in today's world, digital readiness directly impacts health, economic, and social outcomes.

The assessment takes approximately 15 minutes for individual participants to complete. It's designed to be accessible and can be completed on any device—smartphone, tablet, or computer.

Depending on your package, you'll receive:

  • Community summary report with aggregated findings
  • Domain-by-domain breakdown and analysis
  • Action recommendations prioritized by impact
  • Executive summary for stakeholders
  • Implementation roadmap (Premium+ packages)

Reports range from 20-80+ pages depending on your engagement level.

Absolutely. All individual responses are 100% confidential. Reports only contain aggregated, anonymized data—no individual can be identified from the results. We take data privacy seriously and follow best practices for secure data handling.

For statistically meaningful results, we recommend a minimum of 50 participants. Our packages accommodate various sizes:

  • Sponsored assessments: Up to 250 participants
  • Standard packages: Up to 500 participants
  • Enterprise packages: 1,000+ participants

To qualify, your organization must meet all three requirements:

  • Tax Status: 501(c)(3), 501(c)(4), faith-based, or public housing resident council
  • Budget/Community: Under $1M annual budget OR serving community with 50%+ below 80% AMI
  • Mission: Primarily serve LMI families, public housing residents, rural communities, seniors, immigrants, youth, or formerly incarcerated individuals

Plus at least one geographic criteria (HUD QCT, USDA Rural eligible, Opportunity Zone, 20%+ poverty rate, or under 70% broadband access).

Sponsored assessments include the core assessment package: full 36-question assessment for up to 250 participants, community summary report, domain breakdown, and basic recommendations.

Paid assessments offer enhanced features: larger participant counts, more detailed analysis, implementation roadmaps, strategic advisory sessions, multi-year tracking, and executive presentations.

Most courses are delivered live online via Zoom, making them accessible regardless of location. All sessions are interactive with Q&A, breakout rooms, and hands-on exercises.

The TSDOH Practitioner Certification is our most comprehensive program—a 12-week intensive that trains you to conduct TSDOH assessments independently. Graduates receive:
