The TSDOH™ (Technological Social Determinants of Health) Assessment Matrix is a proprietary assessment developed by Dr. Troy Nash that combines a 132-variable community profile with a 36-question resident survey to measure technology readiness across six determinants: Economic Stability, Education Access and Quality, Health Care Access and Quality, Neighborhood and Built Environment, Social and Community Context, and Technology Access and AI Readiness, the 6th determinant.

It is a framework that treats technology access as the sixth social determinant of health, recognizing that in today's world, digital readiness directly impacts health, economic, and social outcomes.

The assessment is designed to take about 15 minutes for individual participants to complete. It's designed to be accessible and can be completed on any device: smartphone, tablet, or computer.

Depending on your package, you'll receive:

• Community summary report with aggregated findings
• Domain-by-domain breakdown and analysis
• Action recommendations prioritized by impact
• Executive summary for stakeholders
• Ranked priority needs and a next-steps brief
• Executive presentation deck, presented live online

Absolutely. All individual responses are 100% confidential. Reports only contain aggregated, anonymized data. No individual can be identified from the results. We take data privacy seriously and follow best practices for secure data handling.

For statistically meaningful results, we recommend a minimum of 50 participants. Our packages accommodate various sizes:

• Sponsored assessments: Up to 500 participants
• Standard packages: up to 500 or up to 2,500 participants
• Large packages: more than 2,500 participants

We'll work with you to design an appropriate sampling strategy for your community.

To qualify, your organization must meet all three requirements:

• Tax Status: 501(c)(3), 501(c)(4), faith-based, or public housing resident council
• Budget/Community: Under $1M annual budget OR serving community with 50%+ below 80% AMI
• Mission: Primarily serve LMI families, public housing residents, rural communities, seniors, immigrants, or youth

Plus at least one geographic criteria (HUD QCT, USDA Rural eligible, Opportunity Zone, 20%+ poverty rate, or under 70% broadband access).

We seek partners among foundations, corporations, healthcare systems, and government agencies to fund assessments for qualifying community organizations. Sponsorship options include:

• Community Champion: sponsors one community assessment
• Regional Partner: sponsors five community assessments
• Transformation Leader: sponsors 20 or more assessments

Sponsors receive impact reports showing communities served, recognition on the AGI Affinity website, and aggregated insights on regional readiness. Learn more about sponsorship.

Sponsored assessments include the core assessment package: full 36-question assessment for up to 500 participants, community summary report, domain breakdown, and basic recommendations.

Paid assessments offer larger participant counts, more detailed analysis, heat maps and executive presentations. The TSDOH™ Strategic Playbook and the Annual TSDOH™ Update are separate engagements.

Both deliver valuable insights, paid packages simply offer more depth and ongoing support.

Speaking engagements are in person: keynotes (45 to 60 minutes), half-day workshops (3 to 4 hours), full-day workshops (6 to 8 hours) and multi-day training (2 to 5 days).

We recommend booking 2-3 months in advance for best availability, though we can sometimes accommodate shorter timelines.

For major conferences or multi-day engagements, 4-6 months advance notice is ideal to ensure scheduling and allow time for customization.
